To Sandra Jeffery Financial Services (A.B.N. 83 002 732 603) and all its related entities.

By applying for this Loan/Hire Purchase/Lease (the “facility”) you agree and consent to the following matters.

1. The words “we” and “us” used in this statement mean Sandra Jeffery Financial Services (A.B.N. 83 002 732 603) and each and every one of its related companies and registered business names (including subsidiaries), whether acting collectively or alone. So, for example, where you are agreeing that we may collect, use or disclose your information for a stated purpose, you are providing your consent to each of these companies and businesses individually as well as collectively.
2. The words “Your Information” used in this statement mean personal information that we obtain about you that we obtain from you or from anyone else regardless of how or when it comes to us.
3. We collect and will collect your information primarily for the purpose of assessing your application for the requested facility and, if your application is approved, for the initial establishment of the facility and then for the subsequent administration and eventual finalization of the facility. You agree and consent to us using your information for this purpose.
4. You also agree and consent to us using and/or disclosing your information for each of the following purposes.
   
   
   1. Helping us to identify a particular lender that may approve the facility herein applied for;
   2. Considering any other application made by you through us;
   3. Helping us to further identify products and services available to us that may be of interest to you (unless you have asked us not to);
   4. Maintaining our relationship with you;
   5. Conducting marketing or customer satisfaction research;
   6. Compliance with legislative and regulatory requirements;
5. Collection of your information is essential for us to service your relationship with us, our panel of lenders and business operation. Without your information that we request you to provide to us, we would be unable to provide you with any of our products or services – including this loan applied for.

5. Your agents, including any stockbroker, legal or financial advisor, or any other finance broker that may be involved in the facility.
6. Your executor, administrator, trustee, guardian or attorney;

8. Regulatory bodies, Government agencies and law enforcement bodies;
9. Any person who introduces you to us;
10. Other parties we are authorized or required by law to disclose information to;
11. Any financier that we may utilize in conjunction with the facility;
12. Any mortgage insurer used by any financier, and any re-insurer used by such mortgage insurer.
13. Sureties and guarantors and any prospective sureties and guarantors. Subject to restrictions imposed upon us by the Privacy Act 1988, you agree and consent to any disclosures by us of your information to an organisation of these types for any of our purposes listed above.

7.  Where any of the information that you supply to us in order to help us provide the facility is sensitive information (such as health status, membership of a trade union or political association.

8.  If you supply us with personal information about another person, you agree to show them a copy of this statement and to tell that person that:

1. We are holding personal information about that person and he or she is entitled to contact us.
2. Personal information collected by that person will be used for the primary purpose set our in this statement and that without that personal information, this purpose cannot be fulfilled.
3. Personal information collected about that purpose will usually be disclosed to third party organizations described in this statement; and
